Who can download the Preventive Plan app?
If you have an iPhone or Android device, you can download the Preventive Plan app in the Apple App Store or Google Play Store. However, the app requires an account sign-up, and only eligible participants employed by a client of U.S. Preventive Medicine can successfully complete registration.
Check with your employer to see if you are eligible to enroll, or reach out to USPM to start offering The Preventive Plan® to your employees.
How do I get started with The Preventive Plan?
If you know you are an eligible program member, download the Preventive Plan app, choose the “Sign Up” option, and enter the requested information so we can match you to an eligible participant in our system. Once you have successfully signed up and created an account, log in for the first time and complete the onboarding survey. This in-depth questionnaire helps us assess your current lifestyle, health risks, and lab and biometric stats — information that will be used to personalize your member care experience. The output of the survey, your Health Report Card, will summarize your results and help you pick a path forward.
Depending on what you want to work on and who you want to become in the new few weeks — a runner, a non-smoker, a more content, stress-free parent or spouse — you will choose your first pursuit. And so your journey begins! Log in every day to consume content crafted by clinical experts, practice small behavior changes that make big impacts over time, and track and share your progress with your Care Team.
I’ve downloaded the app but can’t sign up. What should I do if the app can’t find me?

How do I contact my Care Team?
The USPM Care Team is available via chat, email, or phone. Open the Preventive Plan app and click on the chat icon to send secure messages. To reach the USPM Care Team, you may also use the contact information below:
The Care Team is available Monday through Friday from 8:00 a.m. to 6:00 p.m. Eastern Standard Time.
How do I earn rewards?
Rewards can be redeemed in the in-app Coin Shop. If your current Coin Balance (the number in the upper lefthand corner of the screen) is greater than the coin cost of the reward you want, you can click “Redeem” to cash in your coins and order your incentive.
All rewards and redemption requirements are planned and paid for by your employer. If you don’t see any rewards listed in the Coin Shop, you either have already redeemed the available rewards or your employer has not yet finalized the implementation details for this feature of the wellness program. You will be notified via email when new rewards become available.
Can I connect a Fitbit or other activity-tracking device?
Yes! If you use one of the activity-tracking apps listed below, you can connect your account to The Preventive Plan app to automatically log your workouts, steps, and other activities. Please note that only your physical activity data will show up in the app. Sleep, nutrition, and other health notes will not be recorded or stored. The following apps are available to connect:
After I connect a tracking app, how long does it take for my physical activity data to show up in the Preventive Plan app?
For Apple Health users, your activity data should start appearing within minutes of initial account sync. Any future activity logs should appear in near real-time, but you must open the Preventive Plan app for the entries to appear.
For all other apps, including Fitbit, Garmin, and MapMyFitness, the initial account sync and all future logs should populate in the Preventive Plan app within 24 hours. Be sure to log into the Preventive Plan app regularly to earn your coins!
Where can I schedule my labs or download my physician form?
If your employer has offered lab work as part of your corporate wellness program, you can schedule labs or download a physician form to document a wellness visit through The Preventive Plan app. Open the app on your phone and go to Settings.
Click the option to “Schedule Labs.” The first time you do this, you’ll need to create an account with the lab vendor. Enter your information and log in to schedule lab work or download any forms you may need to complete your employee wellness program requirements.
Can I log what I eat in the app?
Food tracking is a great way to check your nutritional pulse. By recording what you eat and drink, you can get a closer look at trends and patterns in your diet. While there is not a food tracker in this app, there are plenty of apps that specialize in food/drink logging, and you might find the pen-and-paper approach to be just as easy.
After recording your dietary habits for a few days, share your log with your Care Team so they can encourage and offer feedback as you continue to build healthier nutrition habits.
If I abandon a pursuit, do I lose my coins?
No! Once you earn coins for completing a task, those coins are yours to keep. The only way to lose them is to redeem them for rewards in the Coin Shop!
If I abandon a pursuit and then select it again, does my task progress reset?
Yes, each time you begin a new level of a pursuit, whether it’s a first start or a restart, you start with the introductory tasks and work forward from the beginning. Your coins earned during the abandoned pursuit level, however, will not be lost or reset.
