Experience HealthND logoExperience HealthND is a community-based disease management program providing services to Medicaid recipients in the State of North Dakota with chronic conditions including asthma, chronic obstructive pulmonary disease (COPD), congestive heart failure, and diabetes.

 
For past 10 years, Experience HealthND Registered Nurses, who live in North Dakota, have been meeting with recipients face to face and by phone to provide care coordination, education, referral and monitoring to increase self-management and adherence to provider treatment plans.
 
Services are offered in collaboration with providers. Our nurses welcome the opportunity to conduct program orientations and in-services to the practices and facilities we work with. The Registered Nurses fill the vital role of Patient Advocate, while providing care coordination, assessments, extensive education, resource identification, and monitoring. 
 

The program's evidence-based practice guidelines:

  • Increase adherence to doctor's treatment plans and improve self-care practices
  • Close gaps in care and improve quality measures
  • Reduce unnecessary utilization and cost of care
  • Link recipients with appropriate available services and treatment options

Patient Interventions

  • Enrollment and comprehensive assessment
  • Care planning in collaboration with providers, program enrollees and their families
  • Education on the medical home, regular office visits, adherence to the provider's prescribed treatment plan, and their disease(s)
  • Coordination of care for medical needs
  • Assistance to access and utilize appropriate community resources

Program Services

  • Comprehensive assessment, education, and care coordination pertaining to disease condition, co-morbid conditions and risk-reduction preventive measures
  • Care planning in collaboration with providers, recipient, and caregivers
  • 24/7 Telephone Health Information Line for inbound and outbound calls
  • Interpreter services including multiple languages and services for the hearing or speech impaired through the Telecommunications Relay Service (TRS) at 866.713.4852
  • Collaboration with and assistance to access and use appropriate community resources

Healthcare Provider Support

  • Program orientation and ongoing feedback on patient status, progress, and needs
  • Facilitate collaboration between medical providers and community-based organizations
  • Coordination to bridge communication gaps among providers on medical information affecting continuity of care (i.e. ER visit incidence, hospitalizations, RX from various providers)
