How USPM Handles Your Health Information

When you accept the Terms of Use and Consent to Participate, you agree that you have read and understood the Notice of Privacy Practices for Protected Health Information. USPM will have protected health information when you answer questions or provide biometric data.

US Preventive Medicine, Inc. (“USPM,” “we,” “us,” or “our”) Effective July 27, 2026

Welcome to USPM. These Terms cover your use of our websites, apps, and wellbeing programs (the “Services”). By registering for or using the Services, you agree to these Terms and to our Privacy Policy. If you don’t agree, please don’t use the Services.

HIPAA Notice for Participants

US Preventive Medicine, Inc. (“USPM”) Effective July 27, 2026 (replaces the Notice dated November 16, 2022)

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

USPM provides preventive, metabolic, and chronic-condition wellbeing services — like health assessments, lifestyle coaching, and chronic disease management coaching. We don’t provide mental health, substance use, or reproductive health services, and we don’t keep psychotherapy or substance use disorder records. We’re committed to keeping your health information private. “Protected health information” (PHI) is information that identifies you and relates to your health or program — for example, your assessment answers.

Our Role Depends on Your Program

In most programs, USPM works as a business associate — we handle your PHI on behalf of your health plan or employer’s plan (the “covered entity”) under a HIPAA agreement. In those programs, your HIPAA rights come from your plan, so check your plan’s Notice of Privacy Practices to exercise them; USPM helps your plan carry out your requests.

In some programs, USPM is the covered entity. For those, this document is USPM’s Notice of Privacy Practices, and you can exercise your rights directly with us (see below). Your program materials or plan will tell you which role applies.

How We Use and Share Your Health Information

We use and share PHI only as HIPAA and our agreements allow — to support your wellbeing program and coordinate with providers involved in your care, to handle related payment and operations such as quality review, and where the law requires or permits (for example, public health, health oversight, or to prevent a serious threat to safety). We won’t use your PHI for marketing or sell it without your written permission, which you can revoke at any time.

How We Protect It

We use administrative, physical, and technical safeguards consistent with the HIPAA Security Rule, share only the minimum necessary, and require our subcontractors to protect PHI too.

If There’s a Breach

You’ll be notified if a breach affects your unsecured health information — through your health plan when USPM is a business associate, or directly by USPM when we’re the covered entity.

Your Rights (When USPM Is the Covered Entity)

For programs where USPM is the covered entity, you can: see and get a copy of your information; ask us to correct it; request an accounting of certain disclosures; ask us to limit how we use it or to communicate with you confidentially; get a paper copy of this notice; and be told about a breach. Just contact us in writing. For programs where USPM is a business associate, make these requests through your health plan.

Our Responsibilities and Changes

We’re required to protect your health information and to follow the notice in effect for our role. We may update this notice and apply changes to information we already hold; we’ll post the current version prominently and provide it on request.

Complaints

If you think your privacy rights were violated, you can complain to your health plan, to USPM, or to the U.S. Department of Health and Human Services, Office for Civil Rights (www.hhs.gov/ocr/privacy/hipaa/complaints). We won’t retaliate against you for filing a complaint.
